How I  get the personal information and why I  have it

Most of the personal information I process is provided to me directly by you for one of the following reasons:

  • To make an appointment with me, 
  • To fill out the client consultation form, 
  • By contacting me via my website, email or text
  • Diary appointments
  • invoices

I use the information that you have given me in order to obtain the contact. Your information is required to carry out my treatments and to help provide the best possible service to you:

I collect data to ensure we have the right information for assessing your suitability for treatment, for completing the appropriate treatment, for contacting you regarding appointment follow-ups and for a referral to GP or other healthcare practitioners if deemed necessary.

I do not use subscriptions or newsletters, so won't contact you in this form.

I may share your information with other healthcare professionals only if the referral is required, upon your previous agreement.

I may share your information with the rehabilitation software (if used), in order to provide a customised rehabilitation program that is easy for you to access and follow, only upon your previous agreement.

 

Under the UK General Data Protection Regulation (UK GDPR), the lawful bases I rely on for processing this information are: 

(a) Your consent. You are able to remove your consent at any time. You can do this by contacting me

(c) I have a legal obligation -  Your personal information and treatment details will be kept for 8 years for the insurance purposes. 

(d) We have a vital interest.

(f) We have a legitimate interest.

How I  store your personal information 

Your information is securely stored in a locked cabinet, where no one else has access to. 

Your consultation form and treatment notes are kept for 8 years.  I will then dispose of your documents by shredding. In case of treating a minor, the records need to be kept up until their 25th birthday.

Under data protection law, you have rights including:

Your right of access - You have the right to ask us for copies of your personal information. 

Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.

Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances. 

Your right to object to processing - You have the right to object to the processing of your personal information in certain circumstances.

Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
